- an original birth certificate (we will have to hold onto it till August)

REQUIRED DOCUMENTS FOR EACH PARTICIPANT:
(click on the below buttons to download the documents) Registration packets will be provided in person at the Sign-Ups! However, if you would rather print it out yourself and bring it filled out you can!
VERY IMPORTANT: if you want to print your registration packet - make sure you print it double sided- page 2 is intentionally blank. The documents must be printed exactly as in the file or they will have to be filled out again in person.
For both Football & Cheer:
IMPORTANT INFORMATION ABOUT THE PHYSICAL FORM:
This form MUST be completed for each participant. This is the ONLY physical examination form that will be accepted. Physicals cannot be dated prior to April 1st of the year in which the child is participating. Physicals performed by a Chiropractor or Registered Nurse will not be accepted. All physicals must contain the doctor's official office stamp or they will not be accepted.
